Copy of the camper's Physical Examination (within one year)
Any physical form from a physician will work; however, this is an optional form to use Camp Physical Form
Most current Immunization Records or approved exemption letter on file
Proof of Birth, one of the following documents needs to be presented to our front desk
Certified Copy of child’s birth certificate
Birth Registration Card
Notification of birth (hospital, physician or midwife record)
Passport
In addition, if your camper requires medication please complete the following forms as needed. Please note a physician MUST sign the authorization form for long term medications.
