About EIM

We’re not just into 3 letter names, we really do believe that Exercise is Medicine.

As one of 10 formal fitness partners with the American College of Sports Medicine’s global health initiative, Exercise is Medicine®, the Freedom Center has implemented a local, 12-week EIM program consisting of personalized exercise prescription, nutrition education and individual health & wellness coaching.

For the past three years, EIM-FAFC has been providing much needed services to community members who are not meeting the recommended 150 minutes of physical activity a week and who have been identified by their healthcare provider as being high risk for, or diagnosed with, obesity, Type 2 diabetes and hypertension, among other chronic conditions.

Each client starts the program with a Physical Activity and Functional Assessment as well as a biometric screening. Personal trainers and health coaches then work with the client to create a personalized exercise prescription and wellness plan. The EIM program curriculum provides a sense of individual awareness, education and accountability with a team approach.

Integrating the EXI app into the existing, in-person, EIM-FAFC program allows us to expand our services to a greater number of the population whether that barrier is due to geographic location, lack of knowing what to do, or finances. Our team will meet you where you are and support you in your health journey.

Why Deploy EXI?

Research shows even the smallest amount of physical activity can make a huge difference to our health. 

EXI will help support certified professionals and clients / patients on their physical activity journeys. The award-winning platform, comprised of a health professional facing portal and consumer facing app, is also a certified medical device that personalizes a physical activity program for clients / patients, based on current health status. The prescription will be set at the appropriate level and graduated over 12 weeks and compliments the existing, in-person, 12-week EIM-FAFC program.

EXI is personalized for multiple co-morbidities using medical guidelines for 23 long-term health conditions, allowing for weekly goals starting as low as 3 sessions of 10 minutes of low intensity activities per week. Clients / Patients have access to the EXI library of workout videos curated to the appropriate intensity level, or they can log any activity on the app.  EXI gathers health and fitness information from most wearable devices with consent provided in the iOS Health app and the Android Google Fit app. 

Prescription adherence is monitored in real time by health and fitness professionals via the portal using data from the consumer facing app and connected wearable device. This allows for a targeted treatment for sedentary people who are living with, at risk for or diagnosed with chronic health conditions.

The EXI app empowers clients / patients to understand their heart rate zones in real time and to be physically active at the right intensity for them. In addition to the biometrics collected through EIM-FAFC, clients / patients learn important health metrics and how these numbers can improve with exercise, thus, improving their health.

EXI also enables health and fitness professionals to extend training and program personalization safely (based on the intensity prescription) to a wider audience with multiple long-term health conditions. Facilitating accurate and streamlined information sharing between health and fitness professionals is now easier and seamless with the use of EXI’s data portal.
